(For Athletes for NEXT school year)

MAY 9 AT THE HIGH SCHOOL

8th graders - 4:00 p.m.

6th & 7th graders - 5:30 p.m.

In order to practice, play or participate on any Jr High Sports Team you MUST

 have a physical on file at the school.

Forms are in the middle school office and the high school office. 

Parents will need to fill out some items on the physical forms. 

Each student MUST bring that paper with them to the physical.
